Some considerations:
  * This could be an attribute of VISIT (if VISIT becomes synonymous to ENCOUNTER) or 
  * ENCOUNTER (or DATA CAPTURE EVENT) could become a new, optional staging table with optional TRANSFER FROM ENCOUNTER and TRANSFER TO ENCOUNTER and VISIT could be a derived table with standard rules.
  * Rationale for not including within-facility transitions?
