Move the Treatment Proposal to vote. 

Review data examples and finalize the following structural changes:
  * Replace high level Treatment-Condition with FACT_RELATIONSHIP
  * Make relationship to the lower level tables optional
  * Introduce TREATMENT_MODIFIER table to both low and high-level treatments (similar to condition modifiers)
  * Put drug regimen location to vote
  * Put model A vs B to vote
