Handling of admission-discharge transfers (ADT) to CDM

Some considerations:

• This could be an attribute of VISIT (if VISIT becomes synonymous to ENCOUNTER) or
• ENCOUNTER (or DATA CAPTURE EVENT) could become a new, optional staging table with optional TRANSFER FROM ENCOUNTER and TRANSFER TO ENCOUNTER and VISIT could be a derived table with standard rules.
• Rationale for not including within-facility transitions?
